Cornerstone Community Homes has openings for direct support and management staff, all of whom provide assistance in the activities of daily living to our developmentally disabled residents in our seven homes in San Rafael and Novato, and in our community.

Direct Support Professional

The primary role of the Direct Support Professional is to work with Cornerstone residents in the residential setting and community and to assist them as needed in nearly every facet of day-to-day living. Responsibilities include:

  • Working as part of a team to carry out each resident’s Individualized Service Plan
  • Teaching skills to residents by modeling appropriate actions and responses
  • Assisting residents with self-help skills, such as dressing, grooming, and toileting
  • Training residents in the activities of daily living
  • Preparing diet-appropriate and nutritious meals
  • Housekeeping
  • Assisting with self-administration of medication
  • Recording and maintaining various logs and reports

Job Requirements

One to two years of experience in residential programs for the developmentally disabled is preferred, but we also hire and train individuals with no experience. Position requires criminal record clearance and physical/TB test.

The Direct Support Professional pay rate is $11.00 per hour. Candidates with Direct Support Professional-Year 1 Certification start at $11.50 per hour; those with Direct Support Professional-Year 2 Certification start at $12.50 per hour. The overnight rate for all levels is $11.00 per hour for 11 pm to 9 am, with overtime paid per wage and hour regulations.

DAY PROGRAM PROFESSIONAL

The Day Program Professional will work with a Cornerstone client on a one-to-one basis in a community setting to teach and supervise the client with primary consideration to the client’s health, safety, well-being, and personal growth. Responsibilities include but are not limited to the following:

  • Implementing the client’s Individualized Day Program Plan for behavioral and skill development.
  • Teaching and assisting the client in self-help skills such as personal hygiene and grooming, and in independent living skills per the day program plan.
  • Transporting and accompanying the client to planned activities designed to teach community integration skills.
  • Ensuring the good appearance of clients (i.e., clean and appropriate clothing an shoes, good hygiene) in the community.
  • Preparation of appropriate and nutrutious lunch and snacks for the client, which may include client participation.
  • Assisting with the client’s self-administration of medication as prescribed, after completion of the required Cornerstone medication training.
  • Recording daily day program notes.

Job Requirements

  • Criminal record clearance and physical/TB test
  • Three years of direct support experience in programs for the developmentally disabled
  • Direct Support Professional Certification—Year 1 and Year 2
  • Good driving record as verified by a DMV report and insurer approval

The Day Program Professional is a part-time, non-exempt position of 30-hours per week, 9 a.m. to 3 p.m., Monday through Friday, with a pay rate of $14.00 per hour. A full-time schedule may be available in conjunction with morning or afternoon shifts at a Cornerstone residential facility.

ASSISTANT PROGRAM MANAGER

 

The Assistant Manager is responsible for supervising the staff on the weekends, and providing program support weekdays. Position responsibilities include, but are not limited to, the following:

  • Implementation of the residents’ Individual Service Plans
  • Appearance and welfare of the residents
  • Modeling appropriate interactions with residents and staff
  • On-the-job supervision and training of staff in the absence of the Program Manager
  • Daily data collection
  • Assisting with self-administration of medication
  • Housekeeping
  • Menu planning, grocery shopping, and other errands
  • Transportation of residents to doctors’ appointments and outings

Job Requirements

  • Criminal record clearance and physical/TB test
  • Three years of direct support experience in residential programs for the developmentally disabled
  • Direct Support Professional Certification—Year 1 and Year 2
  • Sufficient training hours per regulations
  • Good driving record as verified by a DMV report and insurer approval
  • Must work weekends

The Assistant Program Manager is a full-time, non-exempt direct support position with a pay rate of $15.00 per hour.

PROGRAM MANAGER

 

The Program Manager is responsible for the management of an Adult Residential Facility (ARF) for six developmentally disabled individuals. Responsibilities include, but are not limited to, the following:

  • Implement the residents’ Individual Service Plans
  • Direct supervision and training of direct support staff
  • Communicate with parents and day programs
  • Residents’ medical appointments and prescriptions
  • Quarterly reports
  • Liaison with the Regional Center and Community Care Licensing
  • Schedule staff and meetings
  • Maintain the facility and grounds

Job Requirements

  • Criminal record clearance and physical/TB test
  • Minimum of three years of direct care experience in residential programs for the developmentally disabled, including direct care and staff supervision
  • Direct Support Professional Certification—Year 1 and Year 2
  • Sufficient training hours per regulations
  • Good driving record as verified by DMV report and insurer approval
  • Weekend and after-hours on-call availability
  • Ability to successfully complete 35-hour training course and State test, required for obtaining an Adult Residential Facility Administrator’s License

The Program Manager is a full-time salaried exempt position with partially employer paid medical benefits, vacation, and sick leave.
